• Weight checks
• Tetanus vaccine (TD or Tdap) for adults
• Flu Vaccines (for adults only)

• Medication set-up
• Hemoglobin Tests (For adults only)

Services with fee/physician orders required:

• Blood Draws** (specimens sent out to Quest)- $15 draw fee with insurance (patient must provide proof of insurance)
  • No STAT Labs
  • Appointments required, call 660-258-7251 to schedule an appointment
  • Must have lab order at time of appointment; may be faxed to: 660-258-7105

• Non-vaccine injections (ex: Vitamin B12, Testosterone, etc)- $5/month
  • Physician Orders required/good for 1 year.
• Suture/Staple removal- $5/visit
  • Physician Order required at time of services
• Fasting blood glucose testing- $5/visit
• Pregnancy testing- $5/visit
