Services with fee/physician orders required:

• Blood draws (specimens sent out to Quest)- $15 draw fee
  • No STAT Labs
  • Appointments Scheduled from 0900 to 1200.
  • Must have lab order at time of appointment; may be faxed to: 660-258-7105
    • ATTN: Steven Oakleaf
• Non-vaccine injections (ex: Vitamin B12, Testosterone, etc)- $5/month
  • Physician Orders required/good for 1 year.
• Suture/Staple removal- $5/visit
  • Physician Order required at time of services
• Medication set-up- $5/month
• Fasting blood glucose testing- $5/visit
• Pregnancy testing- $5/visit
